DUTIES AND RESPONSIBILITIES
Provide complete, comprehensive, family-oriented outpatient care for patients throughout the life cycle, including health examinations and treatment of medical conditions, to include follow–‐up and any problems detected.
To manage acute, and chronic illness, develop a plan of care, coordinate and determine the need for a case conference.
Make appropriate referrals to internal disciplines in the center and external agencies for additional services required.
Work closely with the clinical team to provide continuity of care.
Responsible for accurate, timely documentation in the patient’s medical record of patient contacts, case planning and plan of care. All documentation should occur in the patient’s medical record.
Provide clear written and verbal communications to patients and staff, maintaining a culture of dignity and respect.
Participate in an on-call system if needed.
Participate in various administrative/clinical staff meetings to include, but are not limited to Continuous Quality Improvement Committee, general staff meetings, case conferences/management, and/or administrative meetings as requested.
Assist with planning to evaluate performance levels, quality service, productivity and future needs.
Communicate with care managers involved with each patient’s plan of care.
Occasionally participate in public relations functions on behalf of THB Home Health Care LLC.
Handles evaluation and treatment of all chronic diseases including pain management.
Handles patients of all age groups and deals with both acute and chronic diseases.
Examine patients for illness or injury, plan and provide treatment in the form of medicines and therapies. If necessary, refer to:

Dietician
Physiotherapist
Provide education to patients and families regarding medical conditions and their treatment options as well as preventative care strategies of nutrition, exercise, and hygiene
Practice active listening and excellent communication skills with patients, their families, and all staff, addressing patient concerns as the highest priority
Document treatments on digital charting, adhering to THB’s medical charting standards, including ongoing progress notes and test results
Work with a multidisciplinary team (MDT) to collaborate on diagnoses and treatment plan
Order laboratory tests and interprets results
Coordinates with the THB homecare nurses on a daily call for medical updates of the patient’s condition
Coordinates with designated representatives and/or THB homecare patients on a weekly basis to monitor treatment and care progress.
PROFESSIONAL RESPONSIBILITIES:
Adheres to dress code policy, appearance is neat and clean.
Completes annual educational requirements.
Maintains regulatory requirements.
Reports to work on time and as scheduled, and completes work within the designated time.
Wears identification while on duty.
Manages and operates equipment safely and correctly.
Attends departmental meetings, and quality improvement as appropriate.
Complies with all organizational policies regarding ethical business practices.
Communicates the mission, ethics and goals of the facility as well as the focus statement of the department.
PERSON SPECIFICATION
Qualification:
Medical Degree
Experience:
At least five (5) years of medical experience as a GP.
An interest in a home care setting is preferable.
Knowledge/Technical Skills:
Ability to understand and operate necessary computer equipment and software programs.
Personal Abilities/Competencies:
Must be able to receive, interpret and follow verbal and written instructions. The ability to speak a foreign language is helpful, though not necessary. Must be self–‐motivated.
